Nurse Practitioner (NP) - Part-Time Company: Carease Health - IL, a nationwide preventative healthcare company dedicated to reducing hospitalizations and improving patient care. Location: Swansea, IL Job Summary: Carease Health - IL is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team of dedicated healthcare professionals. As a hybrid role, you will have the flexibility to work on-site with our skilled nursing facility patients while also working remotely from home supported by a dedicated scribe team for efficient documentation. Responsibilities: Provide high-quality preventative care to residents in skilled nursing facilities Build strong relationships with facility staff and maintain a patient-centered approach Collaborate with our clinical team to develop and implement individualized care plans Conduct on-site visits and remote work to ensure seamless patient care Utilize remote patient monitoring (RPM) and chronic care management (CCM) to minimize hospitalizations and improve patient outcomes Requirements: Active NP or PA license in the state of practice 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills Ability to thrive in a fast-paced environment with flexibility to adapt to changing priorities What We Offer: Competitive annual salary range: $27,000 - $29,000 Hybrid role with flexibility to work on-site and remotely Support and training from our clinical directors, regional managers, and supervisors Generous paid time off Health insurance for full-time employees Free CEUs Referral bonuses Why Join Us: At Carease Health - IL, we believe in teamwork, compassion, and supporting one another to make a real difference in patient care.
