Education: P… Postgraduate Degree Qualifications Current Physical Therapist license in Minnesota and North Dakota. Graduate of an accredited Physical Therapy program. Current CPR/BLS certification. Valid driver’s license, reliable transportation, and current auto insurance. Ability to travel to patients’ homes. Strong clinical reasoning, communication, and documentation skills. Ability to work independently while collaborating effectively with a multidisciplinary team. Experience in home health preferred. Experience working with geriatric patients preferred. Experience treating neurological conditions preferred. Advanced training or certifications in geriatrics, neurology, balance and fall prevention, LSVT BIG, Parkinson’s care, vestibular rehabilitation, or related specialties preferred. Ability to carry therapy equipment and travel between patient homes. Responsibilities Complete comprehensive physical therapy evaluations in patients’ homes. Develop individualized plans of care based on patient goals, physician orders, and clinical findings. Provide skilled treatment for geriatric, orthopedic, neurological, balance, and mobility conditions. Treat patients with stroke, Parkinson’s disease, multiple sclerosis, neuropathy, brain injury, spinal cord injury, gait instability, weakness, falls, and post-hospital decline. Address balance, strength, endurance, transfers, gait training, safety awareness, and functional mobility. Educate patients and caregivers on home exercise programs, safe mobility, assistive equipment, and disease-specific strategies. Monitor progress and adjust treatment plans as clinically appropriate. Collaborate with physicians, nurses, occupational therapists, speech therapists, caregivers, and other members of the care team. Complete timely and accurate documentation in accordance with Medicare, home health, and company standards. Support discharge planning to promote continued safety and independence.
