Licensed Physical Therapist Assistant

Seeking a Physical Therapist to join a local Skilled Nursing Facility. The role involves collaborating with the therapy teams, evaluating patients, documenting care, and delivering treatment plans to improve patient outcomes. Responsibilities Collaborate with the therapy teams and Rehab Department. Complete patient evaluations and all necessary documentation. Provide recommendations supporting communications and assessments. Provide therapy as outlined in treatment plans and prescriptions. Offer treatments that help improve patients’ limitations and challenges. Independently manage workflow, including time allocation for documentation of individual and group treatments in the electronic records system. Align therapy treatments and plans with an understanding of Medicaid and Medicare billing and PDPM. Qualifications Valid New Jersey Physical Therapy license. Background/experience in long‑term care, specifically a Skilled Nursing Facility. Strong organizational, interpersonal and communication skills.
