Clinical Pharmacist

Organization Outpt Oncology Pharmacy Job Summary Provides leadership in a specialized healthcare team to assure optimal cost‑effective drug therapy. This includes developing pharmacotherapeutic goals, recommending regimens, and developing monitoring plans to meet individualized drug therapy needs of patients. Responsible for leading prevention and reporting of medication errors and adverse drug reactions, and continuously improving processes and practices to enhance safety of medication therapy. Position Highlights Provides critical oversight of safe and effective medication management for the malignant hematology patient population. Oversees chemotherapy/biotherapy medication orders and new treatment plan builds, orders for supportive care medications, assists with chemotherapy treatment selection and dosing, and monitors and manages adverse events. Contributes to financial stewardship, quality and safety initiatives, research and education goals. Works in inpatient and clinic settings. Hours Monday‑Friday 8:00 am‑4:00 pm. Rotating weekend and holiday coverage (≈4 weekends/year). Relocation assistance provided. Preferred Qualifications Hematology experience. Completion of residency training. Key Responsibilities Review drug orders and/or prescriptions to ensure safe, appropriate, and cost‑effective use of medications; prepare and dispense medications; provide drug information; manage medication therapy per regulations and hospital policy. Provide outstanding patient care by optimizing medication preparation and resolving medication‑related problems. Maintain quality, safety, and consistency of clinical pharmacy services in assigned areas. Supervise support personnel and verify accuracy of all medications prepared before dispensing. Additional duties may be assigned. Technical Capabilities Mentoring & Coaching (Intermediate): Demonstrates good training and mentoring practices on a regular basis. Medication Management (Advanced): Demonstrates high expertise in managing medications in complex situations. Pharmaceutical Drug Preparation & Dispensing (Novice): Competent in reviewing, preparing, and dispensing prescribed medications. Pharmaceutical Quality Assurance & Compliance (Expert): Expert knowledge of regulations and continuous quality improvement. Pharmaceutical Computer Literacy (Novice): Proficient with pharmaceutical computer applications. Position Qualifications Certifications: LIC‑Pharmacist License - Tennessee Work Experience: 1 year relevant experience. Education: Bachelor’s degree (required).
