Perinatal Care Coordinator & Referral Navigator

The University of Colorado Anschutz Medical Campus is seeking a Care Coordinator to support PROMISE Clinic in coordinating referrals, scheduling, and patient navigation for pregnant and postpartum patients with behavioral health needs. You will maintain data, liaise with providers, and ensure timely access to services, while protecting patient confidentiality.
