License: LCSW Certifications: BLS, 5150 Training Must-Have: Biopsychosocial assessment Discharge Planning Crisis intervention experience Knowledge of Medicare, MediCal, State, and Federal regulations Experience in linkage with community resources Description:Licensed Clinical Social Worker needed to provide biopsychosocial assessments.Responsible for crisis intervention and short-term counseling.Facilitates connections with resources for patients and families of all ages.Engages in planning for transitions of care.Provides psycho-education and may facilitate support groups.Consults on psychosocial aspects of care as a part of an interdisciplinary treatment team.Serves as a liaison to community programs.Assists with discharge planning in collaboration with multidisciplinary teams.Practices independently to support patients in adjusting to illness.Facilitates goals of care discussions with patients and their families.May provide education to staff and participate on committees. Skills and Knowledge:Strong clinical psychosocial skills with the ability to analyze clinical situations.Knowledge of developmental stages, human behavior, and communication dynamics.Ability to plan and implement effective discharge plans.Understanding of reporting requirements related to abuse. Job Accountabilities:Evaluates all referrals within 24 hours.Initiates therapeutic relationships with patients and families.Conducts psychosocial assessments and treatment planning.Facilitates placement of patients with complex medical and psychosocial needs.Refers patients and families to appropriate care and community resources.Applies interviewing, counseling, and crisis intervention techniques effectively.Maintains accurate and current patient records.Creates and updates a file of community resources for patient linkage.Submits timely productivity and statistical data.Available for scheduled coverage and flexible shifts.Evaluates patients for 5150 holds as necessary.Facilitates group therapy sessions when needed. #J-18808-Ljbffr
