Requirements:

Board Certified or Board Eligible (0-5 years of residency) in OB/GYN - [REQUIRED]
CO license (or willing to obtain) - [REQUIRED]
Clean malpractice - [HIGHLY PREFERRED]
Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - [REQUIRED]
DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - [REQUIRED] * no exemptions
