As we continue to grow, we are seeking compassionate, organized, and motivated Care Managers to support our partner ACOs, MSOs, and physician practices in delivering high-quality population health management services to Medicare beneficiaries. Position Summary The Care Manager plays a vital role in improving patient outcomes by coordinating care, engaging patients, and supporting physicians in managing Medicare populations. This position focuses on proactive outreach, care coordination, and patient education while ensuring compliance with CMS guidelines and organizational quality standards. The ideal candidate has a clinical background, excellent communication skills, and a passion for helping seniors navigate their healthcare journey. Roles & responsibilities Manage an assigned panel of Medicare beneficiaries through ongoing care coordination and patient engagement. Conduct outreach through telephone and secure messaging to educate and support patients. Review and document patient information within Electronic Health Records (EHR). Develop, implement, and update individualized care plans. Coordinate care among physicians, specialists, caregivers, community resources, and ancillary service providers. Support care management programs, including: Chronic Care Management (CCM) Transitional Care Management (TCM) High-Risk Patient Management High-Utilizer Patient Management Utilization Management Monitor patient progress and identify opportunities to improve quality of care and health outcomes. Maintain accurate and timely documentation in accordance with CMS regulations and company policies. Stay current on Medicare guidelines, organizational procedures, and regulatory requirements. Collaborate effectively with interdisciplinary teams to support patient-centred care. Demonstrate professionalism, empathy, and respect while working with a diverse patient population. Qualifications Required Clinical background such as Medical Assistant (MA), Certified Nursing Assistant (CNA), Licensed Practical Nurse (LPN), Registered Nurse (RN), or similar healthcare credential. Foreign trained with Medical Degree welcome Familiarity with medical terminology and care management concepts. Strong verbal and written communication skills. Excellent organizational and time management skills. Proficiency with Microsoft Office, including Excel. Comfortable working with seniors and diverse populations. Ability to work full-time in our Tampa office (this is not a remote position). Preferred Experience in Utilization Management. Experience working with Accountable Care Organizations (ACO), Independent Physician Associations (IPA), Medicare Advantage plans, or physician practices. Bilingual (English/Spanish).
