Per Diem | Night Shift | 8hr shifts | $31.50/hour | Cleveland, OH

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Aristos Rehabilitation and Nursing Center. As a LPN, you will play a vital role in providing high-quality patient care to our residents and exceeding their expectations.

About the Role:

As a LPN, you will be responsible for:

• Providing direct patient care to residents, including administering medications, wound care, and vital sign monitoring

• Assisting with daily living activities, such as bathing, dressing, and grooming

• Developing and implementing individualized care plans in collaboration with residents, families, and other healthcare professionals

• Maintaining accurate and up-to-date records of patient care and treatment

• Participating in interprofessional team meetings to discuss patient care and progress

Responsibilities:

• Patient Care : Provide compassionate and skilled care to residents, including administering medications, wound care, and vital sign monitoring

• Teamwork : Collaborate with healthcare professionals to develop and implement individualized care plans

• Communication : Effectively communicate with residents, families, and healthcare team members regarding patient care and treatment

• Documentation : Maintain accurate and up-to-date records of patient care and treatment
