Why You'll Love This Opportunity Monday - Friday schedule No evenings, no weekends, no call Competitive salary at $141,000 - $147,000 annually (potential for productivity incentive starting year two) Opportunity to help build and grow a new rural health clinic Practice relationship-based medicine with continuity of care Modern electronic health record Strong physician leadership and operational support Structured onboarding and mentoring Career growth opportunities with TeamHealth About the Practice Our rural health clinic provides comprehensive outpatient primary care through an integrated, patient-centered model focused on improving access and health outcomes in rural communities to include Family medicine across the lifespan Preventive care and wellness visits Acute illness and injury care Chronic disease management Women's health Medicare annual wellness visits School, sports, and employment physicals Immunizations Care coordination and population health management Also offers integrated behavioral health services through dedicated behavioral health clinicians This position is focused exclusively on primary care. Behavioral health counseling, psychiatric medication management, and medication-assisted treatment (MAT) services are provided by dedicated behavioral health specialists. The FNP or PA is not responsible for providing or overseeing behavioral health or MAT services. What You'll Do Provide comprehensive outpatient primary care for patients across the lifespan, including infants, children, adolescents, adults, and older adults Diagnose and treat acute illnesses and injuries Manage chronic medical conditions using evidence-based practices Perform comprehensive patient evaluations, histories, and physical examinations Deliver preventive care, wellness visits, immunizations, and routine health maintenance Perform Medicare Annual Wellness Visits and age-appropriate screenings Develop individualized treatment plans and coordinate care with specialists and community resources Educate patients and families regarding disease prevention, healthy lifestyle choices, and chronic disease management Collaborate with the medical director and interdisciplinary care team to deliver coordinated, patient-centered care Maintain accurate, timely, and compliant documentation within EHR You'll Be Supported By Rural health clinic medical director Additional FNP and/or PA Licensed clinical social worker Psychiatric mental health nurse practitioner Licensed practical nurses Medical assistant Patient services representatives Experienced practice manager TeamHealth's operational and clinical leadership teams Qualifications Required Board-certified family nurse practitioner or physician assistant Active Michigan license (or ability to obtain) DEA registration (or ability to obtain) Ability to provide comprehensive primary care across the lifespan Preferred Two or more years of outpatient primary care experience Experience practicing independently in a primary care setting Strong clin Practice Highlights: Autonomy to practice high-quality primary care while benefiting from the support of a nationally recognized healthcare organization Competitive compensation Comprehensive medical, dental, and vision benefits 401(k) with company match Paid professional liability insurance Access to more than 300 CME/CE courses through the TeamHealth Institute Modern electronic health record Comprehensive onboarding and ongoing mentorship Strong physician and operational leadership support Opportunities for career advancement and leadership development
