Job Title & Location Registered Nurse (RN) / Medical Social Worker (MSW) – Case Management Job Details Location: Everett, WA Contract Type: 13-Week Travel Assignment Shift: 4x10 Day Shifts Experience Required: Minimum 1 year Start Date: 06/29/2026 Duration: 13 weeks Hours: 40 hours per week Employment Type: Travel Specialty: LMSW - Licensed Master Social Worker Key Responsibilities Provide comprehensive discharge planning and care coordination for acute care patients Collaborate with physicians, nursing staff, and interdisciplinary teams to ensure continuity of care Manage patient transitions across Med-Surg, ED, and other hospital units Conduct utilization review and support length-of-stay (LOS) management initiatives Ensure appropriate post-acute placement, referrals, and community resource linkage Participate in care conferences and multidisciplinary rounds Float between multiple units including Colby and Pacific campuses as needed Maintain accurate documentation in EPIC EMR system Required Qualifications Active RN License (or MSW depending on role) Minimum 1 year of acute care experience Experience in discharge planning and care coordination Strong background in case management (Med-Surg, LOS management, ED preferred) BLS certification required Benefits Weekly pay Medical benefits #J-18808-Ljbffr LanceSoft, Inc.
