Remote Nurse Case Manager I

Elevance Health (Anthem)

Elevance Health is seeking a Nurse Case Manager I to perform care management for members with complex and chronic needs. This remote/hybrid role supports discharge planning and care coordination, telephonically or on-site as needed, with required in-person training sessions. The position requires a BA/BS in a health-related field and an RN license in applicable states, with multistate licensure if working in multiple states. Bilingual skills are a plus. #J-18808-Ljbffr
