Long Term Care Pharmacist in Sterling Heights, MI

Job Description

This professional pharmacist role supports long-term care pharmacy services, providing essential medication management to patients in compliance with all relevant regulations. Working full-time from 10:00 am to 6:30 pm, you will play a critical part in ensuring the accuracy and safety of medication dispensing as well as collaborating with healthcare providers to optimize drug therapy.

Key Responsibilities:

Verify medication orders and conduct final product checks according to federal, state, and company standards
Perform comprehensive drug utilization reviews to identify and resolve clinical alerts, drug interactions, and therapy issues by communicating directly with prescribers
Maintain meticulous documentation and ensure compliance with all pharmacy regulatory requirements

Qualifications:

Active and clear pharmacist license valid in the state of Michigan
Minimum of 1 year of pharmacy experience demonstrating proficiency in clinical and operational pharmacy functions
Strong attention to detail and commitment to patient safety in medication management
