Case Management Assistant

Your future role at a glance Location: Indianapolis, IN Facility: Ascension St. Vincent Department/Specialty: Inpatient Case Management Schedule: Full time, Monday-Friday, 8:00AM-4:30PM How you'll make an impact in this role Secure service authorizations from insurance providers to ensure patients have uninterrupted access to the medical equipment and specialty care they need for recovery. Coordinate patient transitions by identifying appropriate recovery facilities and home health agencies, ensuring every patient is moved to a safe environment that honors their personal choice. Expedite the discharge process by preparing and delivering essential medical documentation to receiving providers, minimizing wait times and ensuring a seamless hand-off of care. Bridge communication gaps between hospital staff, admissions, and insurance payers to keep the care team informed and resolve any coverage hurdles in real time. Maintain accurate care records by documenting every step of the patient’s journey to provide a clear, high quality history that supports better long-term health outcomes. What minimum qualifications you'll need Education: High School diploma equivalency OR 1 year of applicable cumulative job specific experience required. Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable. What additional requirements you'll need Healthcare experience.
