PT - Physical Therapist

Physical Therapist / Occupational Therapist / Speech-Language Pathologist

This is a 13-week contract position.

AM Shift: Monday to Friday, 8 hours per shift, 5 shifts per week.

We are seeking a dedicated Physical Therapist, Occupational Therapist, or Speech-Language Pathologist for home health services in Marin County. The ideal candidate will provide patient care within a 60-mile service radius and will have experience with Homecare Homebase documentation. The role includes an average of 4-6 visits per day, with a focus on achieving productivity metrics.

Responsibilities:

• Conduct evaluations and develop treatment plans for patients.
• Perform routine visits and assessments, including OASIS documentation.
• Attend weekly case conferences for multidisciplinary collaboration.
• Ensure adherence to productivity standards, accumulating 28-32 points per week.
• Provide support in various territories as needed.

Required Experience / Certifications / Licensure:

• Home Health and OASIS experience required.
• Proficiency in Homecare Homebase documentation is mandatory.
• License as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
