Compensation (1099):

• Start of Care (SOC): $110.00

• Evaluation / Re-Admission: $80.00

• Routine Visit: $70.00

  What You’ll Do as a PT:
  

• Patient Care: Provide high-quality, one-on-one physical therapy as a field PT to geriatric patients in their homes.
  

• Care Planning: Implement effective plans of care to improve patient mobility and safety.
  

• Timely Documentation: Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge) with a clinical team that helps you get it right.
  

• Team Coordination: Collaborate with our clinical leadership team, dedicated office staff, and fellow therapy assistants to deliver outstanding patient care.

What You Need:

• License: Active Florida Physical Therapist (PT) License.

• Certification: Current CPR/BLS Certification.

• Transportation: Valid Driver’s License and reliable transportation.

• Software: Kinnser/WellSky experience is a major plus!
