Actify Home Care has active Geriatric OT home health cases ready for assignment across our expanding territory. We can fast-track your credentialing and get you working quickly.

1099 Compensation:

• OASIS Visit): $80.00 flat rate per completed visit

• Evaluation: $70.00 flat rate per completed visit

• Routine Patient Visit: $65.00 flat rate per completed visit

Territory: North Palm Beach County: Jupiter, Palm Beach Gardens, Riviera Beach, Juno Beach, and surrounding areas.

• Efficient Documentation: We utilize Kinnser (WellSky) for streamlined charting.
  

What You’ll Do as a OT:

• Geriatric Patient Care: Provide high-quality, one-on-one occupational therapy as a field OT to geriatric patients in their homes.

• Care Planning: Implement effective plans of care to improve patient mobility and safety.

• Timely Documentation: Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge) with a clinical team that helps you get it right.

• Team Coordination: Collaborate with our clinical leadership team, dedicated office staff, and fellow therapy assistants to deliver outstanding patient care.
  

What you need:

• License: Active Florida Occupational Therapist (OT) License.

• Certification: Current CPR/BLS Certification.

• Transportation: Valid Driver’s License and reliable transportation.

• Software: Kinnser/WellSky experience is a major plus!
