RN Registered Nurse Case Manager

We are seeking a dedicated and compassionate RN Registered Nurse Case Manager to join our team at Genesee Senior Living in Batavia, NY. As a Registered Nurse Case Manager, you will play a vital role in ensuring our residents receive the highest level of care and support. This is a full-time, Monday-to-Friday position that offers a dynamic and rewarding work environment.

Some on call duties required

Job Summary

Coordinate and manage care for residents across our assisted living facility, ensuring that their needs are met and exceeded. Develop and implement individualized care plans, working closely with interdisciplinary teams to achieve optimal outcomes. Provide direct patient care, including assessments, medication management, and wound care. Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless transitions and optimal care. Consult with families and caregivers to educate and support them in their loved one's care. Participate in quality improvement initiatives to ensuring continuous quality and safety.

Requirements

• Current RN license in the state of New York
• Prior experience in a healthcare setting, preferably in a case management role
• Strong assessment, communication, and problem-solving skills
• Ability to work effectively in a fast-paced environment and prioritize multiple tasks
• Strong attention to detail and organizational skills
