Registered Nurse Case Management

Your future role at a glance Location: Pensacola, FL Facility: Ascension Sacred Heart Pensacola Department: Case Management Schedule: Rotating | Full - Time How you'll make an impact in this role Lead multidisciplinary care planning from admission to discharge, integrating patient preferences, addressing social determinants of health, and arranging post-acute services to ensure safe transitions. Evaluate clinical criteria (InterQual/MCG) continuously to ensure accurate level-of-care assignments, collaborate with payers for timely authorization, and prevent avoidable delay days. Serve as a clinical liaison across care teams, educating staff on thorough medical record documentation while maintaining ongoing dialogue with supervisors and care transition teams What minimum qualifications you'll need Licensure / Certification / Registration: Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required. Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
