Clinical Social Worker MSW or LCSW, Home Health (Outpatient), Full-Time, 8:30 AM - 5:00 PM

The Clinical Social Worker, MSW interviews and evaluates patients and family members to identify social, emotional and economic factors which may interfere with obtaining maximum benefits from medical care; develops and implements a social work treatment plan to be offered to patients and family; selects and utilizes community resources as part of the treatment plan; makes referrals to and coordinates with other professional disciplines or agencies; provides psycho-social therapy using a variety of treatment methods; participates in multidisciplinary health team conferences to formulate the overall patient care plans by interpreting to the team the psycho-social aspects of the patient’s illness; assists in the coordination of the discharge plan. Required Skills Demonstrates skill in patient assessment, intervention, planning, and evaluation. Demonstrates initiative, good judgment, and the ability to make decisions independently. Demonstrates skill in working with patients having planned and unplanned life transitions such as but not limited to diagnosis of chronic and acute illness, death and dying and complex family dynamics. Strong communication skills, verbal, non-verbal and written. Ability to learn department and hospital policies and protocols. Knowledge of managed care payment requirements and Medi-Cal and Medicare requirements. Required Experience Master’s Degree from an Accredited School of Social Work. Current BLS from the American Heart Association (if assigned to Palliative Care, Hospice or Home Health). Evidence of continuing education. Management will track clinical hours. Experience with regulatory requirements related to reporting of child/elder abuse, domestic violence, and other legal requirements. Must possess valid, unrestricted California Driver’s License and valid auto liability insurance. Preferred: Three or more years of work experience in a healthcare setting.
