Requirements

• Degree in Nursing or related field
• Active professional license in the state
• Minimum of 2 years of case management experience
• Strong communication and interpersonal skills
• Experience with electronic medical records (EMR)

Responsibilities

• Assist in the collaboration, development, implementation, revision, and reporting of the case management program.
• Act as a liaison between patients, families, healthcare teams, and other stakeholders to optimize outcomes.
• Coordinate a continuum of care from pre-admission through post-discharge.
• Ensure efficient patient and family progression through hospitalization.
• Consult with the healthcare team on specific patient items.
