Registered Nurse – Case Manager

Shift: Day 5x8-Hour (08:00 - 16:30)

Requirements

• 2-year acute medical Care Manager experience performing Care Coordination/Discharge Planning
• California state license required
• Pending license accepted, but must be licensed upon start date
• No more than 7 days of requested time off (RTO)
• Weekend requirement: typically one full weekend per 4 weeks, subject to department need
• First-time travelers must be screened

Responsibilities

• Coordinate patient care across various healthcare settings.
• Facilitate discharge planning and transition of care.
• Assess patient needs and develop care plans in collaboration with healthcare teams.
• Advocate for patients and ensure access to resources and support.
• Monitor patient progress and adjust care plans as necessary.
• Document all patient interactions and outcomes in accordance with facility protocols.
