Responsibilities
- Lead coordination of patient care with the interdisciplinary care team, ensuring timely and appropriate services
- Serve as a liaison among patients, families, nurses, physicians, and other disciplines to optimize outcomes and discharge planning
- Develop, implement, revise, and communicate the patient’s care plan aligned with clinical progress and coverage of services
- Monitor diagnostic tests, treatments, and procedures; discuss resource utilization and treatment plans with physicians
- Manage complex patients, including high-utilizer ED cases, chronic pain, substance use, and vulnerable populations; collaborate with relationships across departments and with outside providers
- Coordinate transfers to appropriate facilities when needed and ensure continuity of information for ongoing treatment
- Collect data for regulatory and accreditation compliance and contribute to program improvement efforts
- Perform other related duties as assigned

Requirements
- Active RN license in California (state of practice) required
- Basic Life Support (BLS) certification required
- Minimum 2 years of recent experience in case management or a closely related field
- BSN preferred; case management certification preferred
- Experience in an acute care hospital setting with ability to coordinate care across disciplines
- Strong communication, critical thinking, and discharge planning skills

Education
- Bachelor’s Degree in Nursing (BSN) preferred
- Licensure: Registered Nurse (RN) licensure in the state of practice (California) required
- Certifications: BLS required; case management certification preferred
