Social Care Navigator(Temporary Position)- Norwich, NY

The Chenango County Cooperative Extension is seeking two individuals who enjoy coordination work and helping community members. The Social Care Navigator position is an opportunity for someone with social work, human services or healthcare experience. This temporary assignment offers eligibility for NYS benefits, including health insurance, dental and retirement plans. Responsibilities Deliver screening and navigation services in accordance with 1115 Waiver requirements. Complete timely outreach and provision of social care navigation services in coordination with team members. Assist in the development of resource navigation services to meet identified needs. Coordinate referrals platform responsibilities and initiate invoicing for services rendered, maintaining necessary records. Assist the supervisor in making programmatic changes based on evaluation outcomes and local or statewide priorities. Collect evaluation data from program participants and collaborators for program improvement. Interpret evaluation data for reporting and recommend program changes to strengthen services. Perform other duties as assigned. Required Qualifications Computer skills, including e‑mail and time‑collection, and willingness to learn new referral management platforms (primarily Unite Us). Associate’s Degree or high school diploma with 2 years of transferable experience may substitute for a degree. Ability to work independently with minimal supervision and stay organized to complete work within reasonable time. Experience working in a team‑oriented, collaborative environment. Ability to rapidly adapt and respond to changes in environment and priorities. Ability to identify and address safety concerns in the workplace. Ability to work effectively with co‑workers. Ability to communicate with supervisors and staff regarding duties. Ability to follow oral and written instructions and assume responsibility with general guidelines. Ability to plan and organize work responsibilities according to priorities set by the supervisor. Ability to meet travel requirements associated with this position. Ability to work flexible hours, including evenings and weekends, as appropriate. Preferred Qualifications Experience with community members facing challenges. Familiarity and interest in 1115 Waiver goals and procedures. Familiarity with trauma‑informed approaches and interest in advancing health equity through cross‑sector collaboration. Experience managing a caseload of clients and providing 1:1 care management support.
