Your future role at a glance Location: Milwaukee, WI Facility: Ascension St. Francis Hospital Department: Behaviroal Health Social Work Schedule: Days | Part-Time How you'll make an impact in this role Guide seamless patient transitions by creating individual care plans across the full spectrum of recovery—blending clinical needs directly with patient and family values. Resolve critical psychosocial barriers as a subject-matter expert in crisis intervention, legal support, substance abuse, and adjustment needs to protect patient well-being. Advocate for essential resources by proactively identifying community partnerships and systematically removing obstacles to post-discharge care. Partner with interdisciplinary teams through continuous communication to evaluate health plans, optimize medical record documentation, and ensure every patient safe, high-quality care transitions. What minimum qualifications you'll need Licensure / Certification / Registration: One or more of the following required: Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date. Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
