Home Health Caregiver Position Available
Modani Care, a leading provider of in-home care services for seniors and adults, is seeking a compassionate and dedicated Home Health Caregiver to join our team. As a valued member of our team, you will have the opportunity to make a meaningful difference in the lives of our clients, providing personalized support and care in the comfort of their own homes.
About the Role:
As a Home Health Caregiver, you will be responsible for delivering high-quality care to our clients, including tasks such as:
• Assisting with daily living activities, such as bathing, dressing, and grooming
• Providing companionship and emotional support to clients
• Assisting with medication management and vital signs monitoring
• Completing caregiver tasks, such as light housekeeping and meal preparation
Responsibilities:
• Conduct initial and ongoing assessments of clients' needs and develop personalized care plans
• Implement and monitor care plans, making adjustments as needed
• Maintain accurate records of client interactions and care provided
• Develop and maintain positive relationships with clients, families, and healthcare team members
