Responsibilities:

• Provide direct patient care, including administering medications, wound care, and performing vital signs
• Develop and implement individualized care plans in collaboration with interdisciplinary team members
• Monitor and document patient progress, reporting any changes or concerns to the nursing staff and physician
• Participate in shift reports and handovers to ensure continuity of care
• Collaborate with other healthcare professionals to promote a positive and respectful environment
• Maintain accurate and up-to-date patient records

Requirements:

• Current LPN licensure in the State of Vermont
• Ability to work effectively as part of a team
• Strong communication and interpersonal skills
• Professional attitude and commitment to quality patient care
