Job Details Position Summary The Licensed Practical Nurse (LPN) provides nursing care in collaboration with a Registered Nurse, using the nursing process to assess, plan, implement, and evaluate care to maximize patients\' quality of life. The LPN is part of the clinical support team and collaborates with multidisciplinary staff, supporting culture change initiatives and acting as a role model and mentor. Core Competencies Demonstrates empathy and compassion in interactions with patients and families Supports patient independence and choice Strong communication and teamwork skills Commitment to excellence and high-quality care Accountability and personal ownership in patient care Critical thinking and the ability to coach and mentor others Position Responsibilities Ensure care delivered is within safe professional nursing practice Collaborate with mult disciplinary team to meet clinical needs Adhere to best practice and evidence-based interventions Maintain accountability in primary nursing models with RN oversight Support autonomy and patient choice in care planning Document care in designated patient records Identify resources to provide safe and effective care Delegate and supervise nursing assistants Participate in shared governance and care planning initiatives Ensure compliance with HIPAA and regulatory requirements Participate in quality data collection and performance improvement Support unit activities and coach patient care associates Respond to safety concerns on the unit Comply with policies and regulatory matters Qualifications Graduate of an approved nursing program Massachusetts LPN license in good standing Current BLS certification Previous experience preferred Strong communication and organizational skills Proficiency with computer literacy Excellent English language skills Remote Type: On-site Salary Range: $33.00 - $45.00 #J-18808-Ljbffr
