- Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans for residents with acute care needs.
- Implement therapy interventions to improve activities of daily living, mobility, balance, cognition, and upper-extremity function.
- Monitor patient progress, document outcomes, and adjust treatment plans to meet short- and long-term goals.
- Collaborate with an interdisciplinary team including nursing, physical therapy, and speech-language pathology to support discharge planning and continuity of care.
- Educate residents and families on home exercise programs, safety strategies, and adaptive equipment as appropriate.
- Ensure accurate and timely documentation in accordance with facility policies and regulatory requirements. Requirements
