Role Description The Social Worker will provide direct clinical and supportive services to clients in an on-site setting in Hampton Bays, NY. This full-time or part-time role includes conducting psychosocial assessments, developing person-centered treatment plans, and providing individual, group, and family counseling as appropriate. The Social Worker will coordinate care with our PMHNP, therapists, and primary care providers to ensure continuity of care and effective support. Daily responsibilities include maintaining accurate documentation, participating in case conferences, psychotherapy, and supporting crisis intervention and safety planning when required.

Qualifications

• Demonstrated ability to provide clinical social work services, including assessment, treatment planning, and counseling.
  
• Strong skills in rapport building, trauma-informed care, and culturally sensitive, person-centered practice.
  
• Effective written and verbal communication skills, including clear documentation and collaboration with interdisciplinary teams.
  
• Ability to manage a caseload, prioritize tasks, and maintain professional boundaries while working in a fast-paced environment.
  
• Master's degree in Social Work (MSW) from an accredited institution; active or eligible licensure (e.g., LMSW, LCSW) in New York State.
  
• Experience in mental health, substance use, or community behavioral health settings is strongly preferred.
  
• Familiarity with electronic health records and basic computer applications for clinical documentation and communication. xhyhwjd
