Hospice Care Registered Nurse - Fee for Service

About This Role The Hospice Care Registered Nurse provides comprehensive nursing care for patients in a hospice setting, utilizing the nursing process for assessment, diagnosis, and care planning. This role is ideal for experienced nurses who are passionate about delivering compassionate end-of-life care. Highlights Location: Riverhead, NY Schedule: Full-Time What You’ll Do Conduct pre-admission screening and initial nursing health assessments on new patients. Develop nursing plans of care and goals based on patient needs. Provide nursing care, including supervising diet, medication, and activities of daily living. Evaluate the provision of nursing care, including therapy and nutrition counseling. Train and evaluate home health aides and personal care workers. Recommend the rental or purchase of medical equipment and supplies for patients. Provide ongoing evaluation of equipment for defects or additional needs. Participate in the on‑call coverage system on a rotating basis. Engage in the ongoing Quality Improvement Program. Perform related duties as required. Requirements Education Graduate from an accredited School of Nursing; Bachelor’s Degree in Nursing preferred. Experience 2+ years of nursing experience in med/surge, hospice, or home care preferred. Minimum one year of experience in acute care – med/surg strongly preferred. Licenses & Certifications Current License to practice as a Registered Professional Nurse in New York State required. Must be enrolled in an accredited BSN program within two years and obtain a BSN Degree within five years of job entry date. #J-18808-Ljbffr Northwell Health
