Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree from an accredited PT program.
• Current, active Arizona Physical Therapist license required prior to start of assignment.
• Proof of current CPR certification required.
• Current valid driver's license, reliable transportation, and insurance required due to home health travel requirements.
• 1-2 years experience as a Physical Therapist in acute care or rehabilitation setting required.
• Previous home health experience preferred.
• Ability to perform patient assessments, develop and revise Plans of Care, document progress notes, and collaborate with interdisciplinary teams as part of standard practice.
• Ability to safely instruct and assist patients with transfers and durable medical equipment use, and to order/adapt equipment for home environments.
• Ability to perform physical tasks including lifting, bending, pulling, pushing, prolonged standing and walking.
• Ability to work a flexible schedule and travel locally as required by the role.

• Travel Requirements: Must have reliable personal transportation and willingness to travel locally between patient homes.
