Community-Based Support Specialist

This position offers a flexible schedule of 08:00 AM - 04:30 PM.

Job Description

We are looking for a dedicated Community-Based Support Specialist to provide essential support to individuals with Mental Health and Intellectual/Developmental Disabilities (MH/IDD). This is a PRN position, with opportunities to work up to 24 hours per week.

Responsibilities

• Assist and support individuals with mental health challenges and developmental disabilities in a community-based setting.
• Implement person-centered plans tailored to each individual's needs and goals.
• Monitor and document progress in accordance with organizational policies and protocols.
• Collaborate with a multidisciplinary team to foster positive outcomes for clients.
• Provide crisis intervention and support as necessary to ensure the safety and well-being of clients.

Required Experience / Certifications / Licensure

• Experience working with individuals with Mental Health and Intellectual/Developmental Disabilities is preferred.
• A relevant certification or training in mental health or developmental support is desirable.
