The Nurse assumes responsibility for the outcome of nursing care provided to patients in their homes or in facilities. The Nurse demonstrates the knowledge and skills necessary to provide exceptional care for the population served.What You Will DoCollects patient health data.Analyzes the assessment data in determining nursing diagnoses/problem focus.Identifies expected outcomes/goals individualized to the patient.Develops a plan of care that prescribes interventions to attain expected outcomes.Implements the interventions identified in the plan of care.Evaluates the patient's progress toward attainment of the outcomesSystemically evaluates the quality and effectiveness of nursing practice.Collaborates with the patient, significant others, and healthcare providers in providing patient care.Utilizes research/evidence-based findings in practice if appropriate.Considers factors related to safety, effectiveness, cost in planning, and delivering patient care.Essential QualificationsDrivers License - Licensing Board This position requires a valid driver's license and that the employee is insurable by the Avera Health automobile liability insurance carrier Upon HireRegistered Nurse (RN) - Board of Nursing An active license in the state of practice.
