Work Environment

Key Responsibilities

In your role as a Physical Therapist, you will:

• Conduct thorough evaluations by reviewing patient history, performing assessments, and determining treatment recommendations based on your findings.
• Communicate evaluation results and treatment plans to patients and their families, engaging them in the goal-setting process to promote collaboration and understanding.
• Implement treatment protocols tailored to individual patient needs, continuously adjusting based on their progress and the evolving healthcare landscape.
• Consult with multidisciplinary healthcare professionals when necessary to ensure comprehensive care, documenting any changes in the patient's condition that may affect the treatment plan.

You will be working primarily with patients requiring home healthcare services, including those recovering from surgery, managing chronic conditions, or seeking rehabilitation post-injury. Qualifications

The ideal candidate will possess the following qualifications:

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid DPT license in the State of Connecticut.

Preferred Experience
