Field Care Coach: Wellness Navigator (Home Visits)

Humana

Humana seeks a Care Coach 1 in Florida to visit Medicaid members in their homes, ALFs, and LTC facilities, assess needs and create member-specific care plans. You will coordinate services to promote wellbeing and meet member goals while traveling locally up to 90%. Requirements include a Bachelor's degree with 2+ years of health services/related experience or Florida RN/LPN licensure, residency in listed FL areas, and ability to pass language exams in English/Spanish. #J-18808-Ljbffr Humana
