Schedule: Monday - Friday, 8am - 5pmOur CultureOur culture is cultivated using the following values:Integrity: We are honest and professional.Trust: We count on each other.Excellence: We strive to always do our best and look for ways to improve and excel.Accountability: We accept responsibility for our actions, attitudes, and mistakes.Mutual Respect: We treat others the way we want to be treated.QualificationsOn an Average Day You Will:(includes but not limited to)Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely mannerFacilitate the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needsProvide direct spiritual care to patients and families/caregiversServe as liaison and support to community chaplains and spiritual counselorsMaintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revisionDocument direct services and ongoing communication with community chaplains and spiritual counselorsAttend patient care conference and interdisciplinary group meetings as a member of the interdisciplinary groupProvide consultation, education and support to interdisciplinary group on spiritual careRecruit community chaplains, spiritual counselors and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice careProvide for funeral or memorial services for patients as requestedPlan periodic memorial services to meet the needs of personnel, volunteers and community clergy/spiritual counselors working with the hospice teamAssist in supervision of spiritual care volunteers when assigned to patients/families/caregiversActively participate in quality assessment performance improvement teams and activitiesAccept and perform other related duties and responsibilities, as requiredOther duties as assignedRequirements:Graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree preferredMust have experience working with death and dying, individuals/family/caregiverHospice experience preferredWorking knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems.Must be willing to travel to hospice locations as assigned and be flexible with working hoursWe Got the Perks:*Some benefits apply to full-time employees onlyTuition ReimbursementPTO and Paid HolidaysMedical, Dental, Vision, Life Insurance, and moreHSA & 401(k) availableMileage Reimbursement for applicable positionsAdvanced training programsPassionate company culture committed to the highest standard of care in the hospice industryJoin a Team that embraces the reverence of life!EEOC StatementBristol Hospice is an equal-opportunity employer.
