Responsibilities
- Provide excellent patient care while coordinating and reporting on the case management program’s activities.
- Act as a liaison among the patient, family, nurse, physicians, and multidisciplinary team to optimize outcomes.
- Serve as a consultant to the healthcare team on specific patient items and needs.
- Coordinate a continuum of care for a defined patient population from pre-admission through post-discharge.
- Ensure patients and families progress efficiently through the course of hospitalization and discharge planning.

Requirements
- Active RN license in California or eligibility per state requirements
- Nursing degree from an accredited program
- Experience in case management or care coordination preferred
- Strong communication and collaboration skills
- Ability to work in a hospital setting and interact with multidisciplinary teams
