Help patients navigate healthcare, community resources, financial assistance, and other support services. Collaborate with nurses, providers, case management, and the interdisciplinary care team. Assist with discharge planning and transitions of care. Advocate for patient needs while promoting dignity, independence, and well-being. Provide emotional support, education, crisis intervention, and referrals as appropriate. Qualifications Education: Bachelors Degree in Social Work from an accredited school of Social Work Experience: One year of clinical experience in health care preferred, but not required #J-18808-Ljbffr
