WEEKENDS - SOCIAL WORKER (MA/MSW) PRN

Social Worker – Weekends, PRN The Social Worker will participate in the timely assessment of psychosocial issues of patients and their families/support systems while advocating and coordinating a discharge plan and ensuring concurrent appropriate utilization of hospital services, coupled with available community services. Areas may include medication assistance, living wills, health proxies, and other resources as identified. The Social Worker will maintain knowledge concerning regulatory standards (e.g., JCAHO, ACHA) and assist with department compliance. They will provide assistance and expertise with disease support groups and programs within the hospital as needed, recognize signs and symptoms of stroke, call a stroke alert, and verbalize the stroke alert process. They will work with stroke patients and families for discharge planning and treatment based on needs, condition, and prognosis. Stroke knowledge will be maintained through annual 1‑hour online training. Qualifications Education Requirements: Bachelor’s degree in social work or related field is required. Master’s in social work preferred. Experience: 2 years’ acute care hospital experience required. Skills: Microsoft Office, Outlook. License / Certification: NA. Travel Requirements: NA.
