Seven Healthcare are seeking an experienced RN Case Manager specializing in case management to join our team on a contract basis in Clanton, AL. This travel role involves providing comprehensive assessments and care management to high-risk members, ensuring optimal levels of care and health outcomes. The position includes on-call duties for every other weekend, working within the counties of Wilcox, Dallas, Marengo, and Clarke.

Key Responsibilities of the RN Case Manager

• Perform comprehensive assessment of high-risk members.
• Collaborate with primary care providers to implement individualized care plans.
• Address and reassess cost-efficient, appropriate levels of care.
• Implement non-medical support for treatment compliance (e.g., housing, transportation).
• Coordinate care transitions with physicians and social workers.
• Engage specialty resources for optimal resolution of cases.
• Maintain detailed records of clinical, functional, and fiscal outcomes.
• Identify opportunities for health promotion and prevention of illness.
• Act as a patient advocate, protecting privacy and confidentiality.

Requirements of the RN Case Manager

• Associates or Bachelor's Degree in Nursing or related field.
• Current, unrestricted Registered Nurse license in Alabama.
• Experience in Home Health, OASIS, and HCHB.
• Case Management certification.
• Ability to analyze complex medical information.
