Center Medical Director

Role Summary

The Center Medical Director (CMD) at Oak Street Health leads the clinical team to provide optimal patient care and a great workplace environment. The CMD practices primary care 80% clinical and 20% administrative time while serving as a mentor, coach, and advisor to providers, and executing strategic initiatives that align with organizational goals.

Responsibilities

• Support organizational strategy by collaborating with the Practice Manager and Outreach Manager/Director to achieve objectives such as Best Care Anywhere, Unmatched Patient Experience, Exceptional Growth, Great Place to Work, and leadership in DE&I; drive implementation of OSH strategic initiatives.
• Follow direction from regional leadership—regional medical director or senior medical director—to implement regional priorities and meet regional objectives.
• Cascade organizational information to care team members and elevate staff concerns and ideas to the broader organization.
• Coordinate and lead onboarding of new providers at the center.
• Model excellent medical care, implement the Care Model, and support care teams in daily huddles, patient care planning, admission prevention, and clinical documentation workflows.
• Ensure evidence‑based care that keeps patients healthy and reduces hospital admissions.
• Teach and implement Oak Street’s clinical guidelines at both provider and center levels.
• Support preventive care and the value‑based full‑risk model implementation for patients.
• Assess and address root causes when metric goals are not met.
• Create and maintain a culture of coaching, growth, and continuous improvement for providers and care teams.
• Conduct performance reviews, discuss provider goals, and help develop achievable plans.
• Listen to feedback, address concerns with constructive problem‑solving, and celebrate successes.
• Promote positive culture, achieve triad best practices, and support a joyful practice environment.

Qualifications

• MD, DO, APN, or PA with relevant board certification (Internal Medicine, Family Medicine, Family/Adult/Gerontology).
• 5+ years of outpatient practice experience; preferred experience in clinical leadership roles and coaching providers.
• Experience using a metrics‑driven approach to medical care and quality projects.
• Passionate about commitment to older adults and low‑income patients; comfortable with ambiguity and problem solving; remains calm under pressure.
• Fellowship training in Geriatrics, additional professional degrees (MBA, JD, MPH), or prior executive roles are welcomed but not required.
• US work authorization.

Pay & Benefits

Pay Range: $174,070.00 – $374,920.00 (base salary depends on experience, education, geography, and other factors).
