Crouse’s Pharmacy team is hiring a Staff Pharmacist.Available Schedule(s): Part-Time - 28 hours/week 11:00pm-7:30am, 3:00pm-11:30pm with rotating weekends, holidays and other shifts as needPay Rate: $79.00/hourStaff Pharmacist Responsibilities:Completely evaluates all medication orders against clinically appropriate, patient specific parameters.Resolves any identified issues utilizing hospital approved guidelines or with the appropriate provider.Documents all interventions.Initiates substitution of a formulary medication for a non-formulary medication when clinically appropriate.Appropriately documents all clinical activities.Reviews all orders for IV contrast as provided by Radiology.Efficiently utilizes both hospital based and pharmacy based computer systems.Assures that all products are appropriately labeled prior to dispensing.Participates in maintaining a working stock of medications and supplies.Ensures proper storage of medications.Ensures that a patient, nursing unit or department is charged for all medications dispensed/materials utilized.Dispenses a 24 hour supply of unit of use medications to individual patients for all new orders.Checks unit dose carts filled by pharmacy assistants.Checks prepackaged unit dose medications prepared by pharmacy assistants.Staff Pharmacist Requirements:Current New York State Pharmacy LicenseWritten and oral communication skill and interpersonal skills to deal effectively with other health care professionals and hospital employees.Benefits Overview: Medical, Dental, Vision, FSA, and 401K.
