Community Health Navigator
Talentify
Talentify in Rhode Island is seeking a Community Health Worker to connect patients with community-based organizations and to help access health and social services. You will collaborate with the Community Health Team to support care navigation and empower patients. Required are a CHW certification and relevant education; experience with social resources and patient navigation is preferred; basic Windows/Word skills are expected to support documentation and referrals. #J-18808-Ljbffr Talentify
