Community Health Navigator
Brown University Health
Brown University Health seeks a Medical Respite Community Health Worker to support patients facing social determinants of health and coordinate their longitudinal care. You will perform initial assessments, educate patients, and help them navigate both medical and social needs during admission to the Medical Respite Program. The role emphasizes collaboration with hospital staff and external partners, building self-efficacy, and arranging warm handoffs at discharge to ensure follow-up and #J-18808-Ljbffr Brown University Health
