Home Health Aide Patient Escort
Brooklyn Center is actively seeking Home Health Aides to serve as Patient Escorts for our Skilled Nursing Facility in Brooklyn, NY.
Duties:
- Escort residents to and from appointments
- Observing and monitoring residents.
- Reporting any health issues to the supervising nurse.
- Maintain confidentiality of all resident & facility data.
Requirements:
- Valid New York State HHA Certification.
- Occasional heavy lifting may be required.
- Ability to follow the Plan of Care precisely!
- Must be able to work as a team member
- Valid photo ID
