Overview Primary Care (House Calls) Opportunities - Metro Jacksonville, Florida Join a health system that provides tools to support medically complex patients outside the hospital. We are seeking physicians in the Jacksonville, FL area to join our team. Our experience with the IAH Demonstration and our own ACO position us as leaders in Shared Savings. Responsibilities Provide primary care to patients in home-based settings as part of a coordinated care team. Collaborate with a medical assistant and patient care coordinator; utilize dedicated support staff and an EMR with training and support. Conduct approximately 10 patient visits per day with a focus on quality, face-to-face care with patients and families. Support mobile diagnostic services and state-of-the-art laboratory offerings.
