Full Time Professional Watseka, IL, US 5 days ago Requisition ID: 2228 Salary Range: $26.00 To $39.00 Hourly Job Title: Case Management Coordinator Department: Case Management Status: Full Time Schedule: 8am - 4:30pm Summary/Objective The Case Manager provides services to inpatient, observation, emergency department, swing be and resident home patients and is a patient advocate. This role assesses patients on admission for discharge needs and planning including home services, DME, short term and long term placement, financial assistance, emergency assistance, and psychiatric placement. Possess current working knowledge of third party payer guidelines and understanding of inpatient criteria.
