Join Skilled Nursing Pharmacy as a Consultant Pharmacist and play a vital role in enhancing patient care through expert medication management. - Conduct medication reviews and assessments to ensure optimal therapeutic outcomes.
- Provide clinical consultation to healthcare teams and nursing staff on medication-related issues.
- Develop and implement individualized pharmaceutical care plans for patients.
- Monitor patient medication regimens for safety and efficacy, adjusting as necessary.
- Educate patients and staff on medication management and best practices.
- Stay current on pharmacy regulations and industry trends to ensure compliance.
- Collaborate with interdisciplinary teams to improve overall patient health outcomes.
- Participate in quality assurance initiatives and pharmacy audits.
- PharmD degree from an accredited pharmacy school.
- Active pharmacist license in California.
- Minimum of 2 years of experience in a clinical pharmacy setting.
- Strong knowledge of pharmacotherapy and medication management.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a fast-paced environment.
- Detail-oriented with strong analytical and problem-solving abilities.
- Commitment to continuous professional development and education.
