Licensed Social Worker (LSW)

Hopkins Restorative Care Center is looking for a Licensed Social Worker to join our team and play a key role in supporting residents and families from admission through discharge .
With our newly refreshed TCU , you’ll work closely with short-term rehab residents, families, nursing, therapy, and facility leadership to coordinate services, remove barriers, support care planning, and help create smooth, successful transitions.

Minnesota Social Work license required.
Skilled nursing or long-term care experience is preferred.
